New Patient Registrations 


To register we require photographic identification (i.e. passport or a full UK photo driving licence) and proof of your current address ( 2 recent utility bills) You will need to complete a new patient registration form (GMS1) and bring back to reception preferably between 11-7 pm  when reception is not so busy.

Once registered you will be required to have a new patient medical and complete a questionnaire regarding your past medical history.


Patient Communication and SMS Text Service


It is extremely important that the surgery is able to contact patients regarding their health care easily and efficiently.  The surgery will contact patients by means of the contact numbers provided upon registration.  

The surgery will send appointment reminders and SMS texts to patients that have provided a mobile number unless we have been informed otherwise (that you do not wish to be contacted by SMS text).  This is a no reply service unless instructed to reply.  Eltham Palace Surgery will not send any sensitive personal information by SMS.   The surgery may also contact patients by phone on any of the numbers provided.  

We follow this process for the benefit of patient care; to make communicating with our patients as efficient as possible should we need to contact you in regards to your health care. If your contact numbers change, or if you do not wish to be contacted by SMS text message or by phone, please contact the surgery on 0208 294 8150 to inform us of this.



Health Help Now

NHS Greenwich Clinical Commissioning Group have launched a new mobile online service to help people in Greenwich to understand where they should go for treatment, especially when they need health care in a hurry or late at night or at the weekend.


Go to http://www.healthhelpnow-nhs.net/ and select your location as Greenwich on the left hand side if using the Web or by searching in your smart phone app store where you can download the app.

Please bring your blood form and take a ticket and then a seat.
