Please use this form to notify us of any home readings you have taken if you use your own blood pressure monitoring equipment at home.
COVID-19: In light of the current situation, patients are not to come into the surgery for routine health matters. If you require regular blood pressure monitoring, you may be contacted by us and requested to monitor this at home and provide us with the readings. We strongly recommend that patients who need their blood pressure monitored regularly purchase their own equipment for use at home, this will be extremely useful for your long term care and is more reliable than single measurements. Your results will be reviewed and you will be contacted by the surgery if necessary.
Guidance for taking blood pressure readings: Please monitor and record your blood pressure at home for 7 consecutive days (unless you have been advised otherwise). On each day, monitor your blood pressure on two occasions – in the morning (between 6 am and 12 noon) and again in the evening (between 6 pm and midnight). On each occasion take a minimum of two readings, leaving at least a minute between each. If the first two readings are very different, take 2 or 3 further readings.
COVID-19 REMINDER: We are not seeing patients for face to face appointments for blood pressure monitoring and patients are not to use our self service blood pressure machine in our reception, do not come into the surgery. We strongly recommend our patients to purchase their own blood pressure monitoring equipment for use at home, this will be extremely beneficial for use in the long term and is more reliable than a single measurement.
